• Signed/dated VASAP Ignition Interlock Agreement
• TREDS Enrollment Form (ALL SECTIONS MUST BE COMPLETED)
• General Consent Form signed/dated.
• Copy of current court-ordered restricted license or Application of Services if interlock is a DMV requirement.
• A fee agreement must be completed if you are not paying in full for the service. If your case is a DMV interlock only case, the ASAP is permitted to charge a $50 monthly ignition interlock monitoring fee. With a twelve (12) month ignition interlock requirement, the monitoring fees are collected as follows:
  •  The first six (6) months of monitoring fees ($300) are due in full at the time of enrollment.
  •  Before the second six (6) months of monitoring begin, the remaining $300 must be paid in full.
• Consent to Install (if installing on the vehicle NOT registered to you).

Please allow 24 hours for your application to be processed and the authorization for installation to be sent to the vendor.

DO NOT SCHEDULE AN APPOINTMENT WITH THE INTERLOCK VENDOR UNTIL THE REFERRAL HAS BEEN AUTHORIZED BY OUR OFFICE. NO AUTHORIZATION FOR INSTALL WILL BE SENT UNTIL ALL ITEMS ON THE CHECKLIST ARE RETURNED TO THIS OFFICE
